Sentence is either is written in active voice or passive voice.

What is active voice?

When the subject perform the action stated by verb.
For Example
I ate an apple.

What is passive voice?

When the object performed the action started by the verb.
For Example
An apple was eaten by me.

How to change sentence from active voice to passive voice

Step -1: 
Change the position of subject and object. In passive voice subject becomes an object and an object becomes a subject.

Step-2: 

Use appropriate “TO SBE” form of verb related to the object.
Always use the third form of verb in all tenses i.e. past participle.

Step-4: 

Use the word “by” before the subject at the end. Sometimes it may be omitted.

Format of passive voice:

Object + to be verb +past participle of verb +other words (if any) +by +subject.

Form of verb: first form of verb
I speak Hindi.
Form of verb: is/am/are + third form verb
Hindi is spoken by me.
PRESENT CONTINOUS TENSE
Form of verb:
to be + ing form of verb
Viral is painting a card.
Form of verb: is, am, are + being + third form of verb
A card is painted by viral
PRESENT PERFECT TENSE
Form of verb: has/have + third form of verb
She has painted a card.
Form of verb: has/have + been + third form of verb
A card has been painted by her.
PRESENT PERFET CONTIOUNUS TENSE
Form of verb: has /have + been + ing form of verb
No passive voice
SIMPLE PAST TENSE
Form of verb:
Past form of verb
We painted a wall.
Form of verb: was/were + third form of verb
A wall was painted by us.
PAST CONTIOUNS TENSE.
Form of verb:
Was+ were + ing form of verb
 You were making a notes.
Form of verb:
Was/ were + being + third form of verb
A notes were being done by you.
PASST PERFECT TENSE
Form of verb: had+ third form of verb
They had completed the task.
Form of verb: had + been + third form of verb
A task had been completed by them.
PAST PERFECT CONTINUOUS TENSE
Form of verb: had + been + ing form of verb
No passive voice
SIMPLE FUTURE TENSE
Form of verb: will/shall + first form of verb
She will do a work.
Form of verb: will/shall + be + third form of verb
A work shall be done by her.
FUTURE CONTUNIOUS TENSE
Form of verb: will/shall + be +ing form of verb
No passive voice
FUTURE PERFECT TENSE
Form of verb: will have /shall have + third form of verb
I will have saw a movie.
form of verb: will have/shall have + been + third form of verb
A movie shall have been seen by me.
FUTURE PERFECT CONTINIOUS TENSE
Form of verb: will/shall + have + been + ing form of verb
No passive voice


EXCEPTINAL CASES

1) IMPERATIVE SENTENCE

Format:
Let + object + be + third form of verb
For Example
Cook a meal
Let a meal be cooked.

2) “WH” QUESTIONS

“Wh” + to be verb +object + same as normal passive voice
For Example
Why are you making a toys?
Why are a toys being made by you?

3) SENTENCE WITH TWO OBJECTS.

For Example
She guide them in a project.
A project is guided to them by her.
They are guided in a project by them.
In most cases, writing sentences in passive voice is discouraged because it can obscure the subject of the sentence, and confuse the reader. It also usually creates a wordy and awkward sentence construction.
